Fees

Services are provided to the general public at competitive rates. We accept self-pay and are participating providers with several insurance companies. Approved third party payers include First Steps, Vocational Rehabilitation Services, Medicare, and Medicaid. In addition, the Hearing Clinic is a First Steps and Hearing Aid Assistance Program of Indiana (HAAPI) provider.

A sliding fee scale is available for those who meet financial eligibility requirements. Eligibility for reduced fees are based on annual income and expenses.

If you are interested in applying for fee discount, you must submit your federal tax return (e.g., your 1040 or 1040EZ) prior to your appointment. Your written request for consideration must include the name, address, phone number of the patient for whom you are requesting a discount. Please allow up to 10 business days for us to process your request and determine your eligibility.

Payment

The IU Speech-Language and Hearing Clinics accept self-pay and are participating providers with several insurance companies. The clinics are also Medicare, Medicaid, and Vocational Rehabilitation providers. In addition, the Hearing Clinic is a First Steps and Hearing Aid Assistance Program of Indiana (HAAPI) provider. Payment is accepted by cash, check, or credit card. Patient responsibility for payment will be determined by the services provided and the reimbursement offered by any third-party payor.
